Transcript Request

By Mail or Fax

The following information is needed to process an official transcript request:

Full name while in attendance
Date of Birth
Social Security Number
Dates of Attendance and/or Graduation Date
Complete name and address of where transcript should be mailed
$10 – Cashier’s Check, Money Order, or Credit Card (if requesting by fax)

If you desire to pay by credit card, you must access the website and complete the credit card authorization form. Please be sure to sign the Transcript Request and/or the Credit Card Authorization Form

Submit your request by accessing the college’s website, www.livingstone.edu. Click
Academics > Office of the Registrar > Transcript Request > Transcript Request Form
and download the Transcript Request form and Credit Authorization Form. Be sure to include your signature on both forms.

Walk-In

Complete the entire Transcript Request Form
Report to the cashier for payment ($10)
Return to Registrar’s Office with the form and receipt of payment.

No transcripts are released without the written consent of the student. All requests must include your signature
